Original or Photocopy of OR/CR (Official Receipt/Certificate of Registration).
Photocopy of one (1) Government issued ID with signature.

AUTHORIZED REPRESENTATIVE (IF OWNER IS NOT AVAILABLE)

Original or Photocopy of OR/CR (Official Receipt/Certificate of Registration).
Authorization Letter of the owner.
Photocopy of one (1) Government issued ID of registered owner with three (3) specimen signatures.
Government issued ID of the authorized representative with signature.

CORPORATE ACCOUNT

Original or Photocopy of OR/CR (Official Receipt/Certificate of Registration).
Authorization from the company’s authorized signatory.
Photocopy of one (1) Government issued ID with three (3) specimen signatures of authorized signatory.
Company ID or Government issued ID of authorized representative.
